When reporting suspected adverse events, please state:

• What kind of medication the unintended or unfavorable response affects.
• When the adverse event occurred.
• Response to the drug you think is an adverse event.
• If you are a patient, please provide contact on your doctor who has treated you when the adverse event has occurred. Please inform your doctor that we can contact him/her.
• Your contact information (name and surname, phone number, email).
